   Diabetes Prevention


Services

  • Serving enrolled members of a federally recognized tribe
  • Screen Native Americans who may be at-risk for developing diabetes
  • 16-week curriculum focused on nutrition, physical activity and behavior changes
  • Participants reduce their risk of developing Diabetes by reaching a 7% weight-loss and increasing physical activity to 150 minutes per week
  • One-on-one coaching sessions available to each participant as additional support in meeting their weight-loss and activity goals
  • After Core curriculum follows the 16-week intensive curriculum to help participants maintain their goals, continue working towards their goals, or set new goals; group meets once each month until the end of grant funding

  • Community-based screening events, which include education on various health topics
  • Group activity sessions/events
  • Collaborate with annual Diabetes Day Camp
  • Health Fair events

Staff

The Diabetes Prevention or Lifestyle Balance Program consists of the Lead Educator/Dietician, Project Coordinator/Lifestyle Coach, Data Coordinator/Program Assistant, Recruitment/ Retention Coordinator, Grants Manager, Fitness Manager and Lead Diabetes Educator. This is a multi-faceted team, developed to support program participants in successfully reaching their weight-loss and activity goals, with as much support as possible.
