The Program

• BMC is a Community Health Center. Community members can get health care services at a reduced or minimal fee.
• Federal funding allows BMC to charge for services based on income and family size. Visit the Business Office for details.
• Services include: Medical, Dental, Counseling Services, Pharmacy, Lab and Xray.
• You are eligible if you live in the following areas:

• Proof of income is required (Federal Income Tax or Financial Statement)
• Applicants must be screened for Medicaid eligibility

Restrictions

• You must be seen by a BMC provider
• Approved referrals required for: (sliding fee discount may not apply)
• Physical Therapy, Cardiac Rehabilitation, Health Education Services, Nutrition Counseling, Diabetes Management, Environmental Health
• Dental limited to preventive and restorative procedures only. Services requiring lab fees such as crowns, dentures, bridges, etc are not covered.
• Medications are limited to pharmacy drug list only. Patients may receive a written prescription for other medicine or they can pay full price at BMC.
• "Outside" physician prescriptions honored only if a BMC referral.
• Elective procedures such as wart removal, vasectomy, or cosmetic procedure not covered under sliding fee discount.
